            (2)  The name of the pipeline, local distribution company or other
entity receiving the gas downstream of MIGC.
   
        (d)  Term of Service -
   
            (1)  Date service requested to commence; and
   
            (2)  Date service requested to terminate.
   
        (e)  Transportation Rate Requested - Shipper shall specify whether the
service requested is firm(FTS-1) or interruptible(ITS-1) and that it is willing
to pay the maximum Rate provided for the applicable Rate Schedule or, if such
is not the case, will specify the rate at which it is willing to execute a
Transportation Service Agreement.  MIGC shall be under no obligation to
accept requests for transportation at less than the applicable maximum Rate.
